International Travel Information

As a general rule, passports should have at least six months of validity when traveling internationally.

  2. Informed Consent Form
  3. Medical Treatment Authorization
  4. Travel Release
  5. Student Legal Form
  6. Copy of insurance
  7. Copy of your flight itinerary

Please Note: All students will be registered with the USC Overseas department and will be registered for student travel insurance.
