Week 14 Final Case Study Analysis
For this assignment, you will apply what you have learned during the program to create an in-depth case analysis for a real client encountered in clinical practice. Your case analysis will be completed in three sections and will be graded using the case study grading rubric.
Client characteristics
Select a patient of any age that you have seen in clinical practice from one of the following populations:
- Racial/ethnic minority
- Sexual/gender minority
- Veteran
- Pregnant patient
- Immigrant/refugee patient
- A patient with adverse childhood experiences
Remove all identifying information from your evaluation. Your preceptor will be expected to verify the psychiatric evaluation and sign it once complete.
Part A: Cultural Assessment (1–2 pages)
Complete a cultural assessment on your patient. The table below can be used as a guide to write approximately one to two pages discussing your patient’s unique cultural characteristics and recommended interventions using the Culture Care Theory as a guide.

Part B: Psychiatric evaluation (see additional template)
Use the psychiatric evaluation template to complete an evaluation on your patient.
- The evaluation should be completed on a real patient
- Students must use the template provided
- The preceptor must sign and date the evaluation at the end
Part C: Therapy Session (5–7 pages or a 5–10-minute video)
You will document a therapy session held with your client. This does not have to be a formal psychotherapy session; it can also be a meaningful exchange between you The empathy demonstrated should be consistent with the following operational definition: Empathy is a critical tool for establishing a trusting therapeutic relationship. Rather than parrot back what your client has said, good empathy reflects the thoughts and feelings of your client and notes the importance of what has been communicated. In doing so, it invites the client to self-explore. Empathic feedback avoids “why” questions. When appropriately relayed in a tentative manner, good therapeutic empathy also gives the client a chance to redirect or correct what the counselor has said.
For Part C, you can choose from one of the following two options:
- Record a 5–10 minute long video re-enactment of the session with your client. You can have someone off camera speak the lines of your "client" if you choose to do this part of the assignment as a video. It is your choice whether to provide a video of your therapy session or a written transcript.
- Write out your therapy session in a transcript, 5–7 pages
Your session transcript should:
- Use your personal experiences to replicate realistic patient responses as well as clinical responses.
- Video length: 5–10 minutes
- Paper length: 5–7 pages
- Evidence empathic feedback that adheres to the operational definition of empathy in the counselor’s responses to most of the client's remarks.
Part D: The PMHNP Role in Patient Care (4–7 pages)
The role of the PMHNP includes evidence-based interventions involving both psychopharmacological and nonpharmacological services to individuals who have been assessed in a mental health setting. However, the PMHNP also serves as a leader, advocate, promoter of population health, and educator while being cognizant of ethical and legal factors that influence practice.
A. Patient Education
- Consider a moment where you gave a well-worded explanation of the treatment plan to your patient. Write up to one page covering the medication education provided to this client.
B. Evidence-Based Interventions
- Provide 3–5 interventions for your patient rooted in evidence-based practice while making use of appropriate clinical practice guidelines (NICE, APA, VADoD) (1–3 pages)
- Interventions should encompass both medication and non-medication modalities.
C. Ethical/Legal Considerations
Respond to the following prompts in 1–3 pages:
- Describe any legal considerations encountered when caring for this client. What risk reduction strategies will you consider in your own future practice?
- In what way did you assume the role as leader or advocate for this client? How can you serve as a leader in your future role?
- How were ethical concerns addressed in the clinical setting, and how will you address ethical concerns in your future practice?
- Evaluate any facilitators or barriers to care your patient experienced in their healthcare journey. How could the PMHNP address any barriers for future patients?
This assignment should adhere to APA 7th edition formatting and utilize a combination of course materials and literature from the past 5 years. It is encouraged that you break this assignment into chunks and work on it throughout the semester.
