Holistic and Self-Care Plan Case Scenario Part 2

Select the "Begin" button to start.

Introduction

This week, you will continue working on the holistic and self-care plan case scenario from the previous week. This is the second part of Carlos’s visit to the rural community health center. In the first part of the scenario, you examined the patient, completed a holistic needs assessment, and gathered additional information to help you build the management plan. For this week, review your responses from the first part and continue developing the holistic and self-care plan.

Use the information provided and evidence-based resources, along with your acquired knowledge from course materials and clinic experience, to complete the second part of the case scenario. You will be prompted to provide written responses to some questions and have opportunities to choose various alternative answers to work through the case scenario. At the end of the scenario, you will be prompted to save your summary and to submit it for this week’s assignment.

The ultimate goal of the branching case scenario is to provide you with an interactive method of critical thinking needed to manage the needs of a patient.

Let’s continue helping Carlos!

Case Review

In this case study, the patient comes to you for an annual visit. You later find out that he is struggling with his weight, cholesterol, and pain in his joints as a result of the weight. He currently appears to have low self-esteem due to the physical issues he is having. It is important in the holistic care plan to not only address each need but also to be cognizant of cultural beliefs. In your plan, you should include evidence-based responses that support your treatment plan for Carlos.

Conclusion

Congratulations! You have completed the second part of the holistic and self-care case scenario. Print the summary of your responses and save it to your files. You will be required to submit your summary as part of this week’s assignment.

Activity is complete.
